- tear-off portion of the arrival notification of the foreign national at the place of stay, and a copy thereof;

Restoring a Temporary Residence Permit (TRP)
Deadline: 7 days. Where to apply:
- the territorial body of the UVM GU MVD of Russia at the permitted place of temporary residence — if the document was lost within the constituent entity of the Russian Federation where residence is permitted;
- the body at the foreign national’s current location — if the document was lost outside those limits.
Required documents:
- statement of loss of the identity document (in free form);
- two photographs sized 35×45 mm (black-and-white or colour, with a clear full-face image without headwear; for citizens who permanently wear glasses — with glasses without tinted lenses);
- new identity document (if obtained to replace the lost one);
- notarially certified translation of documents into Russian — if the documents are in a foreign language.
Restoring a Permanent Residence Permit (PRP)
Deadline: 7 working days from the date of application. Where to apply:
- the migration affairs division at the place of issuance of the PRP or at the place of residence — if the PRP was lost within the constituent entity of the Russian Federation;
- the migration affairs division at the place of stay — if the PRP was lost outside the constituent entity of the Russian Federation where it was issued or where the citizen has a place of residence (stay).
Required documents:
- application for issuance of a duplicate PRP in two copies (available at the GUVM MVD office, can be printed in advance or filled in by hand or on a computer);
- foreign passport (original) with a notarially certified translation of all pages, including blank ones (the passport must be valid for at least 2–4 or 6 months from the date of application);
- 2 or 4 matte photographs sized 35×45 mm (colour or black-and-white, without retouching; if children were included in the PRP — two photographs per child);
- notification slip from the police (in case of theft);
- damaged document (in case of damage);
- documents confirming changes in personal data (birth certificate, marriage certificate, divorce certificate, etc.);
- tear-off portions of the annual residence notification forms (if available);
- notarially certified translation of documents into Russian (if in a foreign language);
- consular legalisation of documents (for citizens of certain countries).

Restoring a Work Patent
